Monomeric prolactin estimate (MRRL) post polyethylene glycol (PEG) precipitation has been added to the RCPAQAP endocrine program in 2022. The noted flagged low performance compared consensus prompted further investigation identify causes. Prolactin testing by Abbott alinity method showed acceptable excluding any analytical issue with assay. PEG preparation procedure was reviewed and confirmed be correct using 25% solution prepared as 25 g of dissolved water a final volume 100 mL. Repeat all samples new stock mL making 120 (21% solution) an average 12% higher recovery that deemed median group. Moreover, patient comparison 2 preparations 10% MPRL 21% PEG. Based on recent survey conducted RCPAQAP, 60% participating labs are deviating causing improper flagging results.
